The 5-Minute Feedback Form for the KVP Learning Cycle

Many BGM coordinators don't know after a measure whether it actually worked — because they never asked. The effort required for meaningful feedback is minimal, and the benefit for the annual report is enormous.

Why Three Questions Are Enough

The GKV Leitfaden Prävention requires an outcome evaluation for Phase 6 (checks 66–82) that covers three dimensions: process quality (Was the measure well organized?), resource building (Did the measure provide real benefit?), and acceptance (Would participants recommend it to others?).

These exact three dimensions can be captured with three questions — if they're worded correctly.

The Copy-Ready Questionnaire

This questionnaire can be used as a paper slip, a Google Form, or a digital survey tool. Rating on a 1-to-5 scale (stars or numbers):

**Question 1 — Process quality:** 'The measure was organized in a way that let me integrate it smoothly into my daily work.'

**Question 2 — Resource building:** 'I learned concrete methods or exercises to strengthen my health on my own.'

**Question 3 — Acceptance / NPS:** 'Would you recommend participating in this program to your colleagues?'

Evaluation in the Annual Report

The averages across all three questions produce the 'measure score.' For the GKV report: a before/after comparison (if the same measure is repeated) or benchmarking between different measures. A score below 3 on question 2 is a clear signal: the measure isn't producing a lasting effect — reconsider it for next year.
